                                               Citizen's Inquiry Form

When I was on the city council, I developed this communication tool. It was approved and voted on by the city council for use by our citizens.  Citizens please write your concern in the form of a question.

Completing this short form will allow your question to be directed to the correct city department, after being received by the city secretary. The response will be in writing.  A copy of each response is kept in a file for review by council members or others should the same question arise again.

This form should still be available on the front desk at city hall. It may also possibly be sent by fax or email (Go Green! ) upon request.

The requesting party should indicate on the form the preferred method to receive the response.
